Registration Instructions
Student is currently in grades 9-12:
- If the student currently attends a DDSB school and you are moving to the O'Neill area, begin by using the DDSB School Finder to confirm that O'Neill is your home high school. Parents/guardians should contact the Guidance office at their current school to request that they provide a Student Verification Form that reflects the address in O'Neill's area, as well as a status sheet (credit counselling summary) & IEP (if applicable). The school can provide this directly to the O'Neill Guidance Office or the parent/guardian can contact the Guidance Office at 905-728-7531 ext.2 to arrange to send it. After the Guidance Office has received the Verification Form, you will be contacted to arrange a time to bring the following documents: 2 proofs of your address (mortgage statement, lease, water bill, phone bill, cable bill, hydro bill, etc.). After these documents have been verified, O'Neill Guidance will contact your current school to transfer your registration to O'Neill. Finally, an O'Neill Guidance Counsellor will contact your family to discuss course selection.

If the student does not attend a DDSB school, but HAS been registered with the Durham District School Board in the past, use the DDSB School Finder to confirm that O'Neill is your home high school. Parents should contact the office at the last DDSB school attended to request that they provide a Student Verification Form. The school can provide this directly to the O'Neill Guidance Office or the parent/guardian can contact the Guidance Office at 905-728-7531 ext.2 to arrange to send it. We will also require an up-to-date status sheet (credit counselling summary) and IEP (if applicable). After the Guidance Office has received the Verification Form, you will be contacted to arrange a time to bring the following documents: 2 proofs of your address (mortgage statement, lease, water bill, phone bill, cable bill, hydro bill, etc.), updated immunization records and an updated I.E.P. (if applicable). After these documents have been verified, O'Neill Guidance will proceed with your registration. Finally, an O'Neill Guidance Counsellor will contact your family to discuss course selection.

Click this link to complete the online student registration form for 2022-23 or 2023-24 to begin in September (when it opens). Parents should contact the Guidance office at the last school attended to request that they provide an up-to-date status sheet (credit counselling summary) and IEP (if applicable). The school can provide this directly to the O'Neill Guidance Office or the parent/guardian can contact the Guidance Office at 905-728-7531 ext.2 to arrange to send it. After the Guidance Office has received the online registration, you will be contacted to arrange a time to bring the following documents: 2 proofs of your address (mortgage statement, lease, water bill, phone bill, cable bill, hydro bill, etc.), proof of birth date & citizenship (birth certificate, passport, permanent resident card, etc.) updated immunization records. After these documents have been verified, O'Neill Guidance will proceed with your registration. Finally, an O'Neill Guidance Counsellor will contact your family to discuss course selection.
Student is currently in grade 8:
- Students who attend one of our feeder elementary schools are not required to complete any registration steps. They are automatically registered to attend O'Neill and will simply select their courses using My Blueprint. Our Family of Schools include: Beau Valley PS (by address), Coronation PS (Gifted program/by address), Dr. S.J.Phillips PS, Hillsdale PS (by address), Kedron PS, Mary Street PS, Northern Dancer PS, Queen Elizabeth PS, Sunset Heights PS (by address). If you are uncertain about your home high school based on address, contact the office of your elementary school.

There are some students who live in O'Neill's school area but do not attend one of our 9 feeder schools (ie - Walter E. Harris PS for French Immersion). Use the DDSB School Finder to confirm that O'Neill is your home high school. Parents/guardians should contact the office at their elementary school to request that they provide a Student Verification Form that reflects the address in O'Neill's area. The school can provide this directly to the O'Neill Guidance Office or the parent/guardian can contact the Guidance Office at 905-728-7531 ext.2 to arrange to send it. After the Guidance Office has received the Verification Form, you will be contacted to arrange a time to bring the following documents: 2 proofs of your address (mortgage statement, lease, water bill, phone bill, cable bill, hydro bill, etc.). After these documents have been verified, O'Neill Guidance will contact your elementary school to adjust the secondary school to O'Neill. Finally, an O'Neill Guidance Counsellor will contact your family to discuss course selection.

Parents/guardians must go on the O'Neill CVI website to complete the online student registration for 2023-24. After the Guidance Office has received the online registration, you will be contacted to arrange a time to bring the following documents: 2 proofs of your address (mortgage statement, lease, water bill, phone bill, cable bill, hydro bill, etc.), proof of citizenship/birthdate (original birth certificate, original passport, Permanent Resident Card), the most recent grade 8 report card, updated immunization records and an updated I.E.P. (if applicable). After these documents have been verified, O'Neill Guidance will complete the registration. Finally, an O'Neill Guidance Counsellor will contact your family to discuss course selection.
